CHADIS is an online developmental and health questionnaire system that helps the doctor track your child’s development and potential health risks from ages 2 weeks to 5 years. By completing the questionnaire in the comfort of your home, prior to your child’s visit, it allows the doctor to study your responses to the questionnaire before your appointment.
