What is CHADIS?

CHADIS, (Child Health and Development Interactive System) is an online system that provides age-specific questionnaires to support your pediatrician in monitoring your child’s health and development during wellness exams.

How do I get started?

You will receive an email invitation from CHADIS with the details to get your account set up before your next scheduled appointment. Once your account is set up, you can log into your account to view questionnaires that your provider has assigned to you. Questionnaires are assigned to you within two weeks of your next scheduled wellness exam, and you will receive an email reminder from CHADIS each time a questionnaire is assigned to you.

*Please note, if you are unable to complete your CHADIS questionnaire prior to arriving for your appointment, you will be asked to complete it in the office prior to seeing the doctor. 

When should I use CHADIS?

Questionnaires are required to be filled out prior to your child’s wellness exam. It is recommended to complete your questionnaire 1 week before your wellness exam, to give your pediatrician time to review before your visit.

What kinds of questions are asked?

Each questionnaire is different and has a variety of questions that are age-specific. Some questions may require you to interact with your child in order for you to provide the most accurate answer. Here are a few examples of questions in the questionnaires.

Does your child catch a large ball with both hands? (You should stand about 5 feet away and give your child two or three tries before you mark your answer.)
